Social Prescribing
What is Social Prescribing?
Social prescribing connects patients to groups, activities and services in their communities to meet their social, emotional and practical needs that affect their health and well-being. Social prescribing is the bridge between your GP and all the non-healthcare services available in your community.
Who is eligable for help?
If you are an Aldershot registered patient over 18 who needs support, you can refer yourself to our Aldershot Social Prescribing Service.
In general, we can help patients:
• with one or more long-term conditions,
• who need support with their mental health,
• who are lonely or isolated,
• who have complex social needs which affect their well-being.
How does it work?
A social prescribing link worker will contact you and then spend time with you to understand your particular support needs. They will help to prepare a plan for you, so that you can access a range of local support, advice and care providers, groups and activities.
What specific sort of help can you provide?
Your link worker can help you to access many things, including:
Groups and activities for your physical health
Mental health support services and groups
Access to food banks
Help to stop smoking
Access and help with housing services
Medicines management
Befriending services
Accessing Financial advice & support
Access to disability support
Finding hobbies & interests
